The Fallout Anomaly Team has released Fallout World, a survival horror mod for Fallout 4. The mod was previously known as Fallout Anomaly.

A new survival horror mod

Fallout World introduces MAIM, True Damage, and Scourge systems for visceral combat. Enemies can hear almost every player action, a feature inspired by Escape from Tarkov. The vanilla perk system is replaced with a Skyrim-style skill leveling system. The mod also includes hunger, thirst, fatigue, and illness systems. A bespoke weather system built on Polluted Climates is included.

The mod is available globally now.

Fallout World aims to deliver the hardest survival horror experience in Fallout 4. It overhauls core mechanics to increase difficulty and immersion.
